We provide a multi-disciplinary team of senior nurses, paramedics and support workers to avoid unnecessary emergency admissions and A&E attendances of patients resident in care homes. The team includes nurses, paramedics, skilled call handlers and expert administration support with the involvement of pharmacy and GP as deemed appropriate.
The service includes clinical triage and telephone advice (24 hrs a day). It is a quality driven, patient/ client focused, cost-effective service delivered in a timely and appropriate manner against a set of given standards.
We are committed to maintaining a highly skilled workforce utilising current technology to deliver high quality reports which are accurate and that are properly interpreted.
Services also include support to individuals within their own homes.
All staff are clinically supervised by the clinical directors of P2P (senior nurse and paramedic). All staff are trained specifically in infection prevention and control, safeguarding and information governance in addition to their clinical training all staff will be DBS and health checked. The Safer Recruiting Principles are followed.
The service complies with the Essential Standards of Quality (CQC) and the National Quality Standards for out of hours primary-care regarding access, audit and patient satisfaction. We provide weekly and monthly reports regarding activity and performance against the national out of hours standards, complaints, complements, incidents, interventions and referral rates. Record management is provided by Advanced Healthcare’s INurse and community package. Operating on a sophisticated server within a fully NHS compliant data centre. All data is relayed over NHS approved encrypted systems and the NHS’s own N3 network. Patients2People also operate an NHS.net email account for the transferring patient data between data centres and GP computer systems to ensure that GPs are maintained at the heart of a patients care..
